W2: Interactive Case Study - A Coal Mine

Sunday 30 April 2017

Across all industries we continue to have repeat type incidents. We do not seem to learn the lessons of the past. One possible reason for this is hindsight bias - when we know the outcome the decisions seem obvious, and we don't think we would make the same mistakes. The IChemE Safety Centre has developed some interactive case studies to overcome hindsight bias. This workshop will put you in an event as it unfolds, asking you to make the decisions along the way.
